Waiver of Liability and Video/Photo Release

I, {name}, acknowledge and understand that participation in the training programs, classes, and events offered by Roll Society Jiu-Jitsu Academy, LLC involves inherent risks. These risks may include, but are not limited to, accidents, physical injury, illness, or death arising from activities such as falls, physical contact with other participants, or environmental conditions, including extreme heat or humidity. I voluntarily accept full responsibility for any such risks and any resulting injuries or damages.
I affirm that it is my responsibility to disclose any physical or psychological conditions that may affect my ability to safely participate. I represent that I am physically fit and mentally capable of engaging in these activities. I understand that I should consult with a qualified medical professional prior to participation to evaluate any personal risks.
In consideration of being permitted to participate in activities and receive services from Roll Society Jiu-Jitsu Academy, LLC, I, on behalf of myself and my heirs, assigns, and personal representatives, hereby release, waive, and discharge Roll Society Jiu-Jitsu Academy, LLC, including its owners, officers, employees, agents, instructors, representatives, and successors, from any and all claims, liabilities, or demands arising out of or related to my participation. I acknowledge that the academy reserves the right to refuse service at its discretion.
I further grant Roll Society Jiu-Jitsu Academy, LLC, and its assigns, licensees, and legal representatives, the irrevocable and unrestricted right to use my name, likeness, image, video, and/or photograph in any form or media, including edited or composite representations, for advertising, promotional, or any other lawful purposes. I waive any right to inspect or approve the final materials or accompanying text.
By signing below, I confirm that I have read and fully understand this Waiver of Liability and Video/Photo Release. I acknowledge that this document is a binding release of liability, and I voluntarily agree to its terms.
